Request for Funds Use this form when you need FOI to write a check directly to a 3rd party, before the expense has been incurred.
Reimbursement Form Use this form when an expense has already been incurred.
Submit the form with receipts and drop it in the FOI mailbox in the office. Make sure Committee Chair(s) knows about the expenses. Please process expense forms quickly so we can accurately account for each fundraiser. Allow 2 weeks for your check.
